AMENDMENT 1: Attachment 2 - Water Treatment Price List: Row 3, merged columns L, M, N Header Title "Hot Water Boilers - Quarterly" changed to "Hot Water/ Steam Boilers - Quarterly". Row 4 column L Header Title "Hot Water" changed to "Hot Water/Steam". There is no change to the actual frequency.  Attachment 4 - Boiler List: Rows 31 & 32, Column J "Observable System Name" - Changed from "HEATING BOILER - HOT WATER => 750,000 BTU'" to "HEATING BOILER - STEAM => 750,000 BTU" __________________________________________________________________________________________________________ PRE-BID CONFERENCE A pre-bid conference will not be held for this bid. RESPONSIBLE BUSINESS PRACTICES Your organization will perform all the following Responsible Business Practices listed in the section if awarded this contract: Your organization will apply route optimization strategies when providing services, which reduces dependency on oil, saves money and reduces climate changes when feasible. Your organization will, where possible, and in accordance with the scope of services to be provided under this contract, recycle, reuse and reduce waste going to landfill. Your organization will include, where possible, diverse suppliers, vendors and subcontractors when conducting business and in the supply chain. PREVAILING WAGE Do not apply to this Contract.   SERVICE PERIOD Services will be for a five-year period of time. BID BOND A Bid Bond will NOT be required for this solicitation.  PERFORMANCE AND PAYMENT BONDS  Performance and payment bonds will NOT be required for this contract.  COOPERATIVE PURCHASING Other public agencies may establish contracts or price agreements directly with the awarded contractor under the terms, conditions and prices of the original contract; pursuant to ORS 279A.215 and agreement by the selected Contractor to extend the terms, conditions and prices of the original contract. Proposer must sign and return Attachment 3 - Cooperative Purchasing Vendor Agreement located in the Buyer Attachments section of this sourcing event. BID SUBMISSION Bids must be submitted electronically through this Sourcing Event.  Attachment 2 – Complete all the areas highlighted in yellow.  The 'Frequency Cost - Total' is to be the total cost per service. Water Treatment Price List Work Sheet must be completed and uploaded as an attachment to Question 2.1. Transfer the 'TOTAL ANNUAL COST' amount (highlighted in blue)on the Work Sheet to the bid amount for Service Line item S1.1. The spreadsheet will calculate this total for you.  Attachment 3 - Cooperative Purchasing Vendor Agreement must be completed and uploaded as an attachment to Question 2.2. CONTRACT AWARD As a result of this sourcing event process, Multnomah County intends to award a single contract to the lowest bidder who meet minimum requirements.
